High Holidays 5783 (2022) Online Forms

We are excited to welcome our Beth Am members and friends to High Holidays 5783!

Our High Holiday worship will once again be hybrid, both in-person at the Buffalo Grove Community Arts Center and on Zoom.

We are going paperless this year and we are happy to offer you online High Holiday forms and information to complete by Wednesday, August 31, 2022. Should you need assistance completing the online forms or would like an office staff member to work with you on filling out your information online, please don't hesitate to call the synagogue at 847-383-5017.

We know many of you have friends and relatives join us for our special worship services. Non-Beth Am members, adult children (26+) of members, and friends and relatives of members will also be able to purchase tickets for in-person and Zoom worship.

Please complete all the necessary forms below for tickets, Machzorim* (High Holiday prayer books, if you do not already have), Yizkor remembrance names, guest tickets, and accessible parking and seating by Wednesday, August 31.  One submission per household.

Tickets, Machzorim, and a High Holiday gift bag will be available for pick up at Beth Am on: 
Wednesday, September 14 from 5:00pm-7:00pm
Thursday, September 15 from 10:00am-5:00pm
Friday, September 16 from 10:00am-3:00pm
Sunday, September 18 from 12:00pm-2:00pm
Tuesday, September 20 from 10:00am-5:00pm; 6:00pm-7:00pm
Wednesday, September 21 from 10:00am-5:00pm
Thursday, September 22 from 10:00am-5:00pm
Friday, September 23 from 10:00am-3:00pm

If after submitting the completed forms, you have any changes that need to be made, please let Marcy Chez know by September 16th.
